Assistant Practitioner

We are recruiting 22.5 - 30 hours per week depending on preference.
The Assistant Practitioner works as part of the multidisciplinary team. The post holder will support both OT and Nursing Team, to deliver high quality, person centred care to individuals with palliative and life limiting conditions. Under supervision of a registered Occupational Therapist, the post holder provides delegated clinical interventions, supports service users in maintaining function and independence, and contributes to the overall delivery of safe and effective palliative care.
Main duties of the job
* Provide support and interventions for palliative patients.
* Implement, monitor and evaluate individual or group treatment programmes, updating the supervising OT on patient progress.
* Carry out non-complex functional and environmental assessments, identifying needs for equipment or further specialist input.
* Carry out assessments for Hospice at Home nursing service, identifying and implementing ongoing support.
* Prescribe, provide and review standard OT equipment, offering guidance on safe and appropriate use.

Job responsibilities
* To assist the nursing and occupational and therapy teams in supporting palliative patient in their own homes using person-centred interventions.
* Communicate sensitively with patients and families, maintaining dignity, autonomy and respect.
* Work collaboratively with multidisciplinary and multiagency teams to support holistic care planning.
* Maintain accurate, timely clinical documentation, utilising EMIS as the electronic patient record.
* Manage and prioritise own caseload within agreed protocols and professional standards.
* Participate in supervision, appraisal and ongoing professional development, maintaining competence in role requirements.
* Contribute to service development, clinical audit, and the education or induction of colleagues, students and carers.
Experience
* Experience of working with palliative patients
* Community experience
Travel
* Able to travel to required areas of work (Driver own vehicle)
Knowledge and Skills
* Knowledge of principles of Occupational Therapy.
* Able to function as an autonomous practitioner accepting responsibility for delegated caseload with indirect supervision.
* Able to work as a member of the multi-disciplinary team
