Job Summary
We are seeking a dynamic and compassionate Rehabilitation Assistant to join the Rehabilitation Team at Harlington Hospice. The role focuses on helping palliative patients achieve their mobility goals, maximise function, and improve quality of life through expert assessment, treatment, and education.
Key Responsibilities
* Deliver high‑quality, person‑centred care for in‑patients and outpatients in the Wellbeing service.
* Lead the Wellbeing exercise programme and guide patients through personalised exercise plans.
* Provide clinical and administrative support to the Rehabilitation Team, including moving and handling assessments and prescribing equipment.
* Maintain accurate patient records in accordance with policies.
* Empower patients through self‑management strategies and education.
* Collaborate with equipment suppliers, GPs, community nurses, physiotherapists, and other healthcare professionals to arrange smooth and safe discharges.
* Support service development, participate in group sessions, and deliver breathlessness, fatigue, and anxiety management classes.
Qualifications
* 5 GCSE level passes at C or above, including English and mathematics, or NVQ Level 3 Health and Social Care (or equivalent).
Experience
* Experience working in a Health Care Setting.
* Experience working in a multidisciplinary team.
* Experience providing care or treatment interventions directly to patients.
* Knowledge of anatomy and physiology related to the clinical area and of Moving and Handling guidelines.
* Evidence of working at Band 4 level within the NHS.
* Previous experience working with Oncology/Palliative Care patients and supporting with administrative tasks.
