Pay Rate: £36.54 per hour
Hours: 37 hours per week
Hybrid Working: 50% face-to-face / 50% remote

About the Role
Wiltshire Council is seeking an experienced and motivated Occupational Therapist to join the Reablement Team. In this role, you’ll help individuals regain independence and confidence following hospital discharge, enabling them to live safely and independently in their own homes.
As a key member of the team, you’ll carry out strength-based assessments, design tailored reablement plans, and act as an expert advisor on equipment, adaptations, and care planning. You’ll have the flexibility to manage your own caseload while making a genuine difference in people’s lives.
Key Responsibilities
1. Conduct functional and strength-based assessments in community and home settings.
2. Develop and implement reablement plans focused on maximising independence and preventing long-term care needs.
3. Provide expert clinical input and guidance to Reablement Officers and other professionals.
4. Offer advice and recommendations on equipment, assistive technology, and environmental adaptations.
5. Manage your caseload autonomously while maintaining person-centred, outcome-focused practice.
6. Collaborate with hospital and community teams to support timely discharges and transitions of care.

Requirements
11. Degree or Diploma in Occupational Therapy.
12. Current HCPC registration.
13. Minimum 2 years’ post-qualification experience.
14. Full UK driving licence and access to a vehicle.
15. Experience within reablement, hospital discharge, or community-based care.
