Work Type: Full Time

Location: Mount Druitt and Penrith (3 days Penrith, 2 days Mount Druitt)


About us

Wellington Aboriginal Corporation Health Service (WACHS) is an Aboriginal community‑controlled health organisation that aims to empower Aboriginal people to take control of their individual, family, and community health and wellbeing needs through our clinical and community programs.

Our main services are in Wellington, Dubbo, Western Sydney, Penrith, Nepean, and Blue Mountains, and we provide outreach services to other towns and communities through our regional programs.


About the Role

We are seeking a dedicated and culturally aware SEWB (Social and Emotional Wellbeing) Link Worker to provide direct care services. You will be responsible for collaborating with clients, their families, and various services to facilitate access to mental health, community, and other health‑related support systems.


Key Responsibilities

* Deliver care coordination and direct support to SEWB Link clients in collaboration with family, mental health, alcohol and drug services, GPs, RNs, Aboriginal Health Worker, clinical and community partners.
* Develop and manage care plans, providing referrals to health and non‑health services such as housing and community connection.
* Provide transportation for clients, as needed, to appointments, specialist visits, and community support programs.
* Accurately document client data, case notes, and program data, ensuring compliance with reporting and funding requirements.
* Actively participate in staff meetings, case reviews, clinical supervision, and quality improvement processes.
* Adhere to all WACHS policies, legislative requirements, and the WACHS Code of Conduct.
* Demonstrate knowledge and understanding of issues affecting Aboriginal people and communities, and adhere to EEO, WH&S, and Ethical Work Practices standards.
* Perform any other duties as assigned by the Team Leader or Executive Manager.


Qualifications

* A Certificate of Aboriginality.
* Relevant qualifications (e.g., Certificate III/IV in Community Services, Mental Health, AOD Work) or equivalent tertiary qualifications, or working towards one.
* Deep understanding and sensitivity to Aboriginal culture, history, and current issues facing Aboriginal communities.
* Proven experience delivering programs and activities for Aboriginal people.
* Experience in direct care delivery and coordination for mental health, alcohol and other drugs, and community support programs.
* Ability to make appropriate referrals to mental health, AOD services, and community programs.
* Strong organisational and time management skills, with an ability to manage competing priorities.
* Excellent verbal and written communication skills, with the ability to communicate effectively with various stakeholders.
* Flexibility to work across different settings as required.
* Proficiency in using computer software for program data recording.
* Valid Working with Children and Criminal Record checks and a current NSW Driver’s Licence.
