Developmental Community Nurse - Mental Health
Are you a dedicated and compassionate Mental Health Nurse with a passion for working with individuals with learning disabilities?
We are offering a development opportunity from Band 5 to Band 6 for a motivated nurse to join our specialist service supporting people with learning disabilities and complex mental health needs.
This post aims to support your transition from Band 5 to Band 6 through structured professional development, clinical supervision, and mentorship. You will work within a skilled multidisciplinary team, delivering person-centred, holistic care to promote recovery, independence, and quality of life for individuals with learning disabilities and mental health conditions.
The role provides opportunities to enhance skills in care coordination, positive behavioural support, risk assessment, and collaborative working with families and agencies. You will be supported to develop leadership and clinical competencies necessary for a Band 6 role, with regular reviews and tailored learning objectives.
This is an excellent chance for a nurse committed to making a difference and advancing their career in a supportive, specialist environment. Upon successful completion of the development programme and competency achievement, you will progress to a substantive Band 6 position.
Main duties of the job
The Community Nurse assesses, plans, coordinates, and reviews caseloads of adults with learning disabilities, including physical and mental health needs. Responsibilities include liaising with Primary and Secondary health care, as well as private and voluntary agencies.
You will provide clinical support, ensuring the delivery of high-quality, outcome-focused, person-centred services.
This is a generic community care role supporting across Plymouth city localities.
You will participate in reviewing eligibility for Continuing Healthcare funding and make informed decisions in line with national frameworks and local policies.

Qualifications
* Registered Nurse in Learning Disability or Mental Health
* Evidence of ongoing professional development
Additional Requirements
* Ability to travel across Plymouth
* Full driving license and access to a vehicle for work
* Problem-solving and creative skills in care planning
Knowledge
* Understanding of health and social care policies
* Knowledge of the Personalised Care Framework, Mental Health Act, Mental Capacity Act, Deprivation of Liberty safeguards
* Familiarity with safeguarding processes
* Competency in assessment, care planning, and review
* Knowledge of recent developments in learning disability and mental health care
* Understanding of physical health care needs in adults with learning disabilities
Experience
* Working in multidisciplinary teams
