Main duties of the job

Job responsibilities

The successful candidate with be a passionate leader with sound knowledge of end of life care, confidence in care planning and individual assessments. With computer literacy and a resilient and compassionate approach.

Qualifications

* Evidence of continued professional development related to this position
* Advanced physical assessment qualification or willingness to undertake
* Independent prescriber or willingness to undertake
* Sound knowledge of palliative and end of life topics and confidence to practice within the role
* Masters degree in palliative care or similar, or willingness to undertake modules towards achieving
* Degree/degree modules in leadership/management
* Teaching qualification
* Practice assessor and supervisor
* Research experience


Experience

* Substantial post registration experience and expertise in palliative care
* Knowledge of disease processes at the end of life
* Advanced physical and risk assessment skills
* Comprehensive and demonstrable individualised assessment, care planning with advance care planning experience
* Demonstrable advanced communication skills
* Ability to work independently at a senior level and as part of community and extended statutory teams
* Sound community awareness
* Excellent interpersonal skills with the ability to effectively communicate in writing, in person and by telephone
* Listening, negotiating and influencing skills
* Awareness of quality and governance processes
* Experience of developing and teaching others both formal and informal
* Experience of delivering formal education
* Experience of contributing to the development of governance frameworks
* Experience of working with care homes
* Experience of working with people living with dementia and frailty
* A knowledge of hospice and national initiatives relating to palliative care
* Prescribing and medication recommending experience in palliative symptom control and end of life care
