Overview

Hospital at home (H@H) is a short-term, targeted intervention that provides a level of acute hospital care in a patient’s home that is equivalent to hospital care. It includes hospital-level diagnostic tools such as bladder scanners and ECG, and hospital-level treatments such as intravenous medications, oxygen and fluids. The aim of H@H is to prevent admission to hospital and facilitate early supported discharge, focusing on patient choice and personal outcomes.

All staff within the team are required to deliver a high standard of safe and effective medical care within patients’ homes, work autonomously, and recognise and respond to deteriorating patients. They must utilise high-level clinical skills, including venepuncture, ECG, IV additives and cannulation, with excellent communication and time-management skills essential for these roles.

Valid NMC registration and a flexible attitude to work, with the ability to travel within the area in a timely manner, are essential. The post requires flexibility to work over a 7-day period, covering shifts from 08:00 to 22:00 hours. Continuous Professional Development is supported, and staff are encouraged to attend regular supervision and undertake annual appraisals.

A requirement of this post is to become a member of the Protection of Vulnerable Groups (PVG) Scheme prior to appointment. Qualifications and requirements

* Willingness to work a 7-day shift pattern (08:00–22:00).
* PVG Scheme clearance prior to appointment.
* Right to work in the UK; ability to demonstrate eligibility as part of pre-employment checks.
