We are seeking autonomous and innovative practitioners to join our Integrated Community Mental Health Teams (CMHTs) in Bradford. Our service is committed to delivering recovery-focused interventions, supporting individuals on their personal recovery journey through creative and flexible approaches.
You will work as part of a multidisciplinary team including Doctors, Nurses, Psychologists, Social Workers, Occupational Therapists, and Support and Peer Workers. Together, the team delivers care aligned to our Care Closer to Home model, ensuring accessible and person-centred support within the community.
Main duties of the job
* Responsible for the delivery of community mental health care to a caseload of clients in accordance with the objectives of the Care Trust and within team operational policies.
* Ensure implementation of all Trust Policies and Procedures as required.
* Receive and accept referrals for the service and initiate, lead and co‑coordinate their assessment, making referrals to other professionals as required.
* Manage a personal caseload of clients.
* Conduct assessment, planning, intervention and evaluation of care for a designated client group. Undertake accurate, timely and skilled assessments and ongoing reassessment of client condition, taking into consideration all social, physical and environmental factors.

Qualifications
* Registered Mental Health Nurse, Occupational Therapist or Social Worker
* Evidence of post registration training in Mental health needs/ challenging behavior
* Diploma in Nursing
* FLiCP/SLiP (or equivalent)
Experience
* Experience of working, or placement in a Community Mental health setting
* Experience in relevant area of Mental Health Nursing.
