Job Description About Me Hello I am a 10 year old boy and I live at home with my dad, grandmother, grandfather, two uncles, one auntie and younger cousin. My dad is married but my step mom does not yet live with us. I am described by my family as a strong minded, very happy, active boy who enjoys cuddles, playing with my noisy toys, listening to Disney nursey and seeing all my friends and teachers at school. Although I do not have any siblings, I have cousin's who visit me regularly and I always look forward to their visits. I was born with Spastic Cerebral Palsy, which has in turn impacted on my growth and development. I have Profound Global developmental delay which means in comparison with other 10-year-olds, I am behind with my developmental milestones and will rely heavily on your support. I am unable to verbalise however I can understand you and I am good at letting you know what I like and dislike through my facial expressions and body language. When I am happy, I laugh and make coo-ing noises. When I am sad, I will cry. I also experience dystonia, spastic quadriplegia and scoliosis which have impacted on my mobility, I am therefore wheelchair bound, have restricted movement in my limbs and I am unable to control my head movements. I have a body brace which I should use each day too. My grandmother says I used to have frequent seizures but they have been under control for several years, my medication is effective at present. Unlike you, I am unable to take any food orally and rely on my gastrostomy for medication and feeds as I used to experience severe gastro- oesophageal reflux which resulted in fundoplication and gastrostomy installation. I used to also have tasters of easy to swallow foods, such as yoghurt, but currently this is on pause due to recent events of increased vomiting. I experience frequent daily vomiting which puts me at risk of aspiration and developing chest infections. I am prone to developing both as well as gastrostomy site infections if my PEG is not cleaned daily and water in my balloon is not changed once a week. Please support me by working along my dietician’s plan so I may in turn achieve my targeted growth and weight. Nutrition is also important for me as it will impact on other areas of my wellbeing such as overall health and my skin. I do enjoy spending time with my family too and participating in my cultural and spiritual celebrations with the support of my family, may you kindly be respectful of my needs, which I am sure you will do. Lastly, please support me in creating a school and home routine within a safe environment so I can develop and be cared for safely.
