We have an exciting opportunity for a dedicated Social Worker to join the Home First Hospital Team in North Lincolnshire. Responsibilities: You will complete information gathering on the wards. You will hold MDT (multidisciplinary team) conversations with professionals to decide the pathway for a safe and timely discharge. Additionally, you will be able to respond to A&E and plan for care from the hospital.
