Job Description:
Under the leadership and management of the Team Manager/Principal Social Worker, you will provide a statutory social work service to adults with health and social care needs. Your responsibilities include conducting high-quality, timely, and person-centred needs assessments that reflect individual needs and risks, along with analysis, care planning, intervention, and review. You will work collaboratively with colleagues from different teams, disciplines, and partner organizations to deliver a seamless service to individuals, families, and carers.
You will work with individuals facing complex social and family crises across various settings, promoting autonomy and development, especially for those with complex needs and vulnerability to exclusion. Additionally, you will collaborate with communities, individuals, and families to promote their personal strengths. All work must be carried out in accordance with Adult Social Care practice standards, the quality assurance framework, and the national Professional Capabilities Framework (PCF) for Social Work.
* The Social Worker will maintain and enhance their knowledge and skills, contributing to the learning and development of team members. Professional conduct should reflect the values of the department and the council at all times.
* You will stay informed about social policy and practice, working within the relevant legislative context. You will undertake all social work tasks within prescribed timescales and standards, prioritizing the welfare of adults.
* You will actively pursue professional development, contributing to supervision, appraisal, and training opportunities, ensuring compliance with HCPC requirements.
* You will collaborate with team members and multi-disciplinary teams from other agencies to meet the needs of vulnerable adults, functioning effectively within the operational team.
* You will ensure the views and wishes of individuals and carers are central to decision-making processes.
* You will act to protect adults in need of support, following legal requirements and safeguarding policies of the council.
* You will promote independence, prevention, re-ablement, and self-determination, facilitating effective communication with families, carers, individuals, and other agencies.
* You will work within the council’s budgetary framework and understand legal limits related to funding streams like Continuing Health Care.
