Community Occupational Therapist
Adult Social Care – Community Assessment Team | Nottingham
An exciting opportunity has arisen within Adult Social Care for two Community Occupational Therapists to support citizens across Nottingham through in-person assessments and reviews within community settings. This role focuses on enabling individuals to maintain independence, safety, and wellbeing within their own homes through comprehensive assessment and intervention.
The successful candidates will conduct face-to-face reviews and assessments in line with the Care Act and Mental Capacity Act, ensuring individuals receive appropriate support, equipment, and interventions that promote independence and reduce risk.
This position offers a hybrid structure with community visits and one office-based day per week, supporting effective case management, collaboration, and service delivery.
The role holder will deliver a responsive and person-centred service to individuals, families, and carers, ensuring that people are supported to live safely and independently while receiving the right level of care and support.
Main Duties:
1. Undertake in-person occupational therapy assessments within community settings
2. Complete Care Act assessments and reviews to determine care and support needs
3. Carry out Mental Capacity Act assessments where required
4. Conduct risk assessments and develop appropriate intervention plans
5. Assess for and recommend assistive technology, equipment, and adaptations
6. Provide guidance to individuals, families, and carers to promote independence and safe care practices
7. Ensure all reviews are conducted face-to-face within the community (no telephone reviews)
8. Work collaboratively with multidisciplinary teams, care providers, and partner agencies
9. Maintain accurate case records and documentation in line with professional and organisational standards
10. Contribute to delivering an efficient, responsive, and person-centred community occupational therapy service
Requirements:
11. Diploma/Degree in Occupational Therapy
12. Current HCPC registration as an Occupational Therapist
13. Experience completing Care Act assessments and reviews
14. Knowledge and experience applying the Mental Capacity Act
15. Experience in manual handling assessments, risk assessments, and assistive technologies
16. Strong assessment, communication, and problem-solving skills
17. Ability to work independently within community settings
18. Full UK driving licence, access to a vehicle, and willingness to travel both inside and outside the council area as required
