Job Description Practice Nurse Organisation: Chorlton Family Practice Location: Manchester Hours: Full-time (37.5 hours per week) / Part-time considered Reports to: Practice Manager / GP Partners About Us Chorlton Family Practice is a friendly, progressive GP surgery based in South Manchester, serving a diverse local population. We are proud to deliver safe, effective and compassionate primary care, supported by a multidisciplinary team of GPs, nurses, pharmacists, HCAs, and administrative staff. We are now seeking an experienced and motivated Practice Nurse to join our clinical team. Job Purpose To provide high-quality, evidence-based nursing care to patients registered at Chorlton Family Practice. This includes long-term condition management, treatment room duties, health promotion, immunisations, and supporting the delivery of QOF, IIF, and enhanced services. The role requires working collaboratively with the wider clinical and administrative team to ensure excellent patient outcomes and a safe, effective service. Main Duties and Responsibilities1. Clinical Care Manage and provide care for patients with long-term conditions such as diabetes, asthma, COPD, and hypertension. Undertake chronic disease reviews, medication checks, and titration in accordance with local and national guidelines. Perform cervical cytology, childhood and adult immunisations, and travel vaccinations. Carry out treatment room duties including phlebotomy and administration of therapeutic injections (e.g. B12, Zoladex, Prostap). Provide health promotion and lifestyle advice on smoking cessation, weight management, diet, and exercise. Support womens and mens health services as required. Maintain high standards of infection prevention and control in all aspects of clinical practice. 2. Health Promotion and Public Health Deliver patient education to promote self-care and healthy lifestyle choices. Participate in national and local health campaigns (e.g. flu, NHS Health Checks). Identify and respond to safeguarding concerns for both adults and children in line with practice and statutory procedures. 3. Teamwork and Communication Work collaboratively with GPs, ANPs, pharmacists, HCAs, and admin colleagues to ensure a coordinated approach to patient care. Participate actively in clinical meetings, audits, and quality improvement projects. Maintain accurate, contemporaneous clinical documentation in accordance with NMC standards using EMIS Web and Ardens templates. Support effective handover and communication within the team. 4. Professional Development Maintain NMC registration and comply with revalidation requirements. Complete all mandatory and role-specific training, including BLS, infection control, and safeguarding (Level 3). Participate in annual appraisals, reflective practice, and clinical supervision. Engage in continuous professional development to maintain and enhance clinical skills. Qualifications and Experience Essential Registered Nurse (RGN) with current NMC registration. Experience working within a primary care or community nursing setting. Competent in chronic disease management and general treatment room duties. Up-to-date training in immunisations, cervical cytology, and basic life support. Familiarity with EMIS Web, Ardens templates, and QOF processes. Excellent communication, interpersonal, and organisational skills. Commitment to ongoing professional development and teamwork. Desirable Additional qualifications in chronic disease management (e.g. asthma, COPD, diabetes). Experience delivering travel health advice and vaccinations. Mentorship or teaching qualification to support HCAs or trainee nurses. Personal Attributes Compassionate, patient-focused, and proactive in approach. The role involves regular use of computer systems and clinical software. Adherence to infection control, confidentiality, and data protection policies is mandatory.
