What you'll be doing
* Provide safe and secure medicines management and reconciliation service to the agreed standards.
* Manage prescriptions and interventions, providing face-to-face advice to patients.
* Provide continuity of care to patients upon discharge.
* Build strong relationships within the multidisciplinary team.

Requirements
* Registration with the General Pharmaceutical Council.
* Up-to-date with Continual Professional Development (CPD).
* Relevant post-qualification pharmacy experience.
* A driving licence is essential.
* Experience in a hospital, community, or prison environment is preferred.
Additional information
* Must have the legal right to work in the UK.
* Offers are subject to vetting and security checks.
* Work pattern: 4-week rolling shift, long days for 1-3 weeks, followed by 1 short week.
* Working hours: Monday to Friday 07:45 - 17:30; Wednesday to Friday 07:45 - 15:00.
* Salary up to £43,534 per annum, depending on experience.
