The post holder will work as part of the Palliative Care Respite Team and will be responsible to the Team Coordinator. The post holder will assist the District Nursing Team in delivering high quality community based nursing and social care to patients who have palliative care needs and who wish to remain at home. The post holder will work collaboratively with other members of the Intermediate Tier Services in supporting the patient and carers and provide personal, social, psychological and spiritual care. He/she regularly works without direct supervision in a patient's home.
