Bank Community Neurorehabilitation OT's. The Bexley Community Neurorehabilitation Team is looking to appoint motivated and committed Specialist Occupational Therapists with neuro experience essential. Ideally you will be an existing Band 6 OT seeking to develop specialist skills in neurology and community working; applications from experienced Band 5s are welcome for potential consideration. The team provides a service in clients' own homes, care homes and the local community with opportunities for joint working, joint goal setting and regular in‑service training. You will work with support from the 8a Clinical Specialist and Highly Specialist Band 7 OTs to provide specialist neurological OT assessment and treatment to clients with highly complex neurological conditions including Stroke, MND, MS, Head Injury and Parkinson's. It is essential that you hold a full UK driving license and have access to a suitable vehicle for business purposes.
This is a Bank role (work on an as and when required basis). You will be part of a busy community multidisciplinary Neuro-Rehabilitation team in the London Borough of Bexley, with flexible working supported by the trust. The service encourages continued professional development with in‑service training and opportunities to work closely with colleagues in the South East London sector.
Main duties
* To provide specialist occupational therapy input to clients as part of the specialist Community Neurorehabilitation service.
* To work with specialist nurses and other health and social care practitioners to manage those with neurology long term conditions.
* To supervise Band 5 occupational therapists/ therapy assistants and occupational therapy students.
* To be responsible and accountable for assessment, diagnosis and treatment for a wide variety of conditions including complex cases, in the community as an autonomous and lone practitioner to clients.
* To demonstrate specialist knowledge levels and skills in neurology conditions and interventions.
* To demonstrate theoretical knowledge of multiple pathologies and impairments in the assessment of community clients, often in the presence of incomplete or conflicting referral information.
Responsibilities (continued)
* To undertake effective support, guidance, supervision, and appraisal of junior staff; identify development areas for the Occupational Therapy service and communicate with the Lead therapist.
* To provide training and education programmes for allied health professionals, rehabilitation assistants, social care assistants and other care trust staff via tutorials and presentations.
* To maintain continuing professional development through regular in‑service training, supervision and reflective practice, and disseminate learning to the team.
* To utilise communication and teaching skills to educate diverse populations in disease‑specific therapeutic techniques, principles and guidelines.
* To assess for and provide equipment to enhance independence in daily living skills; monitor, review and re‑assess complex care plans against outcomes to modify care packages as needed.
* To carry out clinical risk assessment and complete risk management plans; seek guidance from more experienced colleagues for highly complex presentations.
* To work in a coordinated, multidisciplinary manner and communicate effectively with the wider clinical network involved in client care.
* To provide advice and education to clients, carers and relatives based on clinical evidence; attend case conferences and contribute to decisions on client care and wellbeing.
* To prioritise caseload against team guidelines and standards; work with specialist neurology nurses and Neurorehabilitation Manager to develop services for neurology long term conditions.
* To identify and manage mental / emotional health needs of the client with guidance from senior staff as needed.
* To maintain up‑to‑date clinical written records using trust systems; write discharge and progress reports to referrers.
* To manage a clinical caseload as part of the MDT; support development of evidence‑based OT practice; contribute to service development and risk management.
* To provide specialist OT advice to the MDT; maintain competency in manual handling and ensure quality standards for occupational therapy are continually improved.
* To identify development areas for the Specialist Community Neurorehabilitation team and work with senior therapists and team lead to develop them; stay abreast of evidence‑based and best practice; participate in clinical audits as required.
* To flexibly plan and organise own time and that of therapy assistants; manage daily caseloads and other professional duties; be flexible in undertaking other duties as requested by line managers.
* To attend and participate in Specialist Community Neurorehabilitation team or Trust meetings and case conferences as required; comply with professional guidelines, codes of conduct, Trust and departmental policies; maintain punctuality and reliable attendance.
