Job Title: Band 5 Community Nurse
Location: Kincardine and Mearns District Nursing Team, covering Portlethen, Stonehaven, Laurencริมkirk and Inverbervie. Primary base to be confirmed.
Hours: 22.2 hours/week, 08:30–17:00, Monday – Sunday.
A position has arisen for a Band 5 Community Nurse within the Kincardine and Mearns District Nursing Team, covering Portlethen, Stonehaven, Inverbervie and Laurencekirk. Primary base to be confirmed.
Are you a passionate and dedicated registered nurse looking to make a real difference to the lives of patients in their own homes and local communities? We are seeking an enthusiastic and skilled nurse to join our dynamic community healthcare team.
Responsibilities
* Deliver high-quality, person-centred care to patients within the community, helping them to manage long term conditions, recover from illness and live independently in their own home.
* Work closely with a supportive multidisciplinary team to ensure patients receive the best possible care in the comfort of their own environment.
* Deliver a range of nursing interventions, including wound care, chronic disease management, medication administration and palliative care.
* Care Planning: Develop, implement and review care plans for patients in collaboration with the DN/Senior Community Nurse.
* Multidisciplinary Collaboration: Work as part of a team with doctors, care management and other healthcare professionals to ensure holistic, patient-focused care.
* Clinical Skills: Undertake clinical tasks such as injections, dressings and monitoring of vital signs.
Requirements
* A registered nurse with a current NMC registration
* A genuine passion for providing care in the community setting and supporting patients to live independently.
* Strong clinical skills, particularly in wound care, medication administration and palliative care.
* Excellent communication skills and the ability to build relationships with patients, families and team members.
* Ability to work independently and manage your own caseload.
* Full driving licence and own car.
