As the Team Manager for the Hospital Social Work Team, you’ll lead a dedicated group of professionals supporting patients with complex needs through safe, effective and timely discharge from Nobles and Ramsey Hospitals.
Responsibilities
* You’ll work closely with ward-based multidisciplinary teams, the Complex Discharge Team, and the Access and Capacity Team to streamline care pathways and improve the experience for patients and their families.
* Guide a well-established team through a period of positive change as Manx Care shifts the balance of care from hospital to community.
* Inspire collaboration, support innovation, and drive improvement to enhance patient experience and recovery.
* Lead a service that puts people first and supports meaningful change across the Islands health system.

Person Specification
Essential
* Social Work Professional Qualification (e.g. BA/MA Post Graduate Diploma in Social Work, Dip. SW, CQSW)
* Management Qualification Award (or willingness to pursue one)
* Registered with Social Work England
