Job Overview
This is a post in primary care within the community mental health team in Denbigh.
We provide assessments from numerous agencies who refer through our SPOAA daily meetings, and offer brief interventions within groups and occasionally on a one-to-one basis.
Having a good knowledge of third sector agencies is useful, as we work to become more familiar with and signpost to these helpful organizations, continually improving our working relationships.
Main duties of the job
An exciting opportunity has arisen in Primary Mental Health Support services for an experienced, motivated, and forward-thinking practitioner to join a busy supportive team based in Denbighshire.
The successful candidate will work closely with a multidisciplinary team, receive referrals directly from GPs, and provide short-term interventions for individuals with mild to moderate illnesses. They will collaborate with colleagues from secondary care, specialist services, third sector organizations, and statutory agencies.
They will manage a caseload, work on risk management plans, and develop care plans based on thorough assessments. We seek a team player with excellent communication skills, and the ability to prioritize and manage time effectively.
Desirable: Ability to speak Welsh; Welsh and/or English speakers are equally welcome to apply.
