Job Opportunity: Community Palliative Care CNS (Band 7)
An exciting opportunity has arisen for an experienced Palliative Care Clinical Nurse Specialist (CNS) to work within the community setting. We are seeking a skilled, autonomous, and innovative nurse to join our integrated specialist palliative care nursing team.
Role Overview
This is a Band 7 community-based palliative care CNS position.
You will work alongside Community Nursing Services, Consultants, Therapists, and the wider multidisciplinary team (MDT).
Your role is to ensure the delivery of high-quality, patient-focused care, providing advice and support to patients, families, and healthcare professionals.
You will collaborate closely with GPs, community teams, and acute trust staff.
You will support community teams to confidently and competently deliver palliative and end-of-life patient-centered care and provide expertise for patients with palliative care needs across the borough.
Essential Qualities and Skills
Enthusiasm, motivation, compassion, and leadership skills.
A commitment to and progressive work within palliative care.
Excellence in clinical practice, acting as a resource for information and education.
Ability to work alongside other specialist services in a challenging environment.
