Job Summary
Clinical Pharmacy in General Practice is part of a programme of transformation to develop a new model of care that delivers person-centered, coordinated care across the PCN. The model aligns with national policy, including the Five Year Forward View and GP Forward View, to better utilise pharmacy roles within primary care to help patients stay safe and well and reduce demand on general practice. Pharmacy technicians work under supervision to support pharmacists and other team members, and are not able to prescribe or make clinical decisions. Their core responsibilities cover clinical, technical and administrative activities.
Location: Tudor Surgery, Nantwich, Cheshire. Part of Nantwich and Rural PCN, serving approximately 35,000 local patients, with a focus on professional development in a supportive environment.
Main duties
An exciting opportunity to work within Tudor Surgery, undertaking clinical, technical and administrative roles to support the Clinical Pharmacist in delivering clinical pharmacy services. You will perform medicines improvement work to maximise safe, cost-effective prescribing under supervision, assist with medication safety monitoring systems (e.g. high-risk drugs), liaise with clinicians and administrative staff to resolve medicine-related queries, monitor prescribing policies, support Structured Medication Reviews (SMRs) and medication reconciliation, and undertake patient-supporting roles to promote effective medicines use. The role will develop over time as the post holder gains experience.

Job Description
Job responsibilities
* Support the role of our Clinical Pharmacist in the delivery of clinical pharmacy services.
* Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the GP or Clinical Pharmacist.
* Assist with medication safety monitoring systems (e.g. high-risk drugs).
* Liaise with clinicians and administrative staff to resolve medicine-related queries.
* Monitor practice prescribing and local and national prescribing policies.
* Improve service and quality through audits.
* Support the Clinical Pharmacist in Structured Medication Reviews (e.g. organise necessary monitoring tests before SMR).
* Undertake patient-supporting roles to ensure effective medicines use through shared decision-making.
* Support medicine reconciliation for patients transferring back into primary care, liaising with patients and other providers to ensure appropriate post-discharge medication.
* Carry out medicines optimisation tasks including administration, reviews and medicines reconciliation on transfer of care.
* Assist the Clinical Pharmacist with searches, data collection and medicine safety tasks.
* Increase uptake of Electronic Repeat Dispensing and the Electronic Prescription Service.
* Support the Practice to deliver on QOF and locally commissioned enhanced services.
* Implement changes to medication from CAS/MHRA alerts, product withdrawals and other guidance.
* Work collaboratively with the clinical team and compare with community, hospital and MMT pharmacy colleagues and the wider health system.
* Participate in practice meetings, patient participation groups and other local meetings to improve engagement with the role of a pharmacy technician.
Person Specification
Essential
* Professional Registration with the General Pharmaceutical Council (GPhC)
* BTEC/NVQ Level 3 or equivalent in pharmaceutical science/pharmacy services
* Evidence of continued professional development
* Understanding of clinical knowledge and evidence-based healthcare
* Experience of dealing with a range of clinical conditions
* Experience in primary care, community or hospital settings
* Good clinical pharmacy knowledge and terminology
* Advanced knowledge of Primary Care Networks, General Practice and evidence-based medicine
* Awareness of systems to support management of patients in a primary care setting
* Understanding of current healthcare policy within the PCN and NHS targets
* Planning and prioritising workloads to meet deadlines
* Excellent verbal and written communication skills with the ability to adjust style to the audience
* Ability to manage allocated resources and interpret prescribing data
Desirable
* Minimum of 2 years post-qualification experience
* Experience with EMIS
