Responsibilities
* To work as a skilled practitioner and be responsible for the delivery of patient-focused programmes of care, addressing the complex and changing health and well-being needs of the client group in your caseload.
* To work with other professional groups in secondary and primary settings to develop and maintain holistic services for patients.
* To operate within a multidisciplinary team to care for patients and their families, delivering high service standards and sharing best practice.
Qualifications and Experience
* We are looking for experienced, motivated and ambitious nurses who want to work in a friendly, supportive and innovative environment.
* The post will be offered either at band 6 or 7 depending on experience. If the successful applicant starts at band 6 they will be supported to complete ACCEND capabilities in practice and to work towards a postgraduate qualification in order to progress to band 7 status, providing autonomous nurse-led services alongside the rest of the team.
