Description The Service & Team: The Rough Sleeper Service works to prevent people from sleeping rough and to help them sustain their accommodation. Where someone does end up sleeping rough, the service intervenes to try and ensure that this episode is rare, brief and non- recurring. Based within a Rough Sleeping Accommodation Programme (RSAP) Team, as part of the wider Rough Sleeping Service, the post holder will work with people as they move on from rough sleeping into accommodation with support through to independence. They will support the individual in their recovery to move from the street into accommodation, access support to address personal needs, find employment, undertake training, develop budgeting skills, cultivate positive social circles and be better equipped to maintain their accommodation and independence and avoid returning to the street. Support needs will range from low to very complex and including, but not limited to, drug and alcohol addiction, offending history, poor mental health, financial difficulty and physical health problems. Support will be offered on site-based accommodation and in homes dispersed within the community. The post-holder will work in a way that is creative and innovative, agile and flexible, and solution-focused, tailoring their approach to the needs of the person. They will employ a trauma-informed and psychologically informed approach, building on the individual’s strengths and will seek to reduce harm. The post-holder may be required to support people in a Housing First Tenancy. The core principles of Housing First are: People have a right to a home Flexible support is provided for as long as is needed Housing and support are separated Individuals have choice and control An active engagement approach is used People are supported to identify their strengths, goals and aspirations A harm reduction approach is used This is a public/customer-facing role, where the statutory English language requirement for public sector workers applies. This position will be subject to a enhanced criminal record disclosure check. Working Pattern: 37 hours per week, flexi working Monday-Friday 9-5pm What you’ll need to succeed: Support clients by giving choices and information, providing an enabling service so that they can access appropriate services, minimise harm and overcome barriers to engagement Support the completion and review of risk assessments and support needs assessments with clients, assisting in the delivery of actions identified within Support Plans Identify the complex needs of often highly vulnerable and high-risk clients, which will include substance misuse and abuse, mental health issues, offending and violent behaviour, anti-social behaviour and individuals involved in the sex trade, and supporting the referral process to relevant services Encourage and assist clients to establish positive social networks and undertake meaningful activities to assist and promote social inclusion Support clients to engage with their recovery, develop life and social skills, claim benefits and access appropriate accommodation, avoiding a return to rough sleeping Build effective relationships with a wide range of accommodation providers to ensure clients are able to move into accommodation that is suitable and appropriate to their needs Provide a multi-agency response and communicate information between all relevant agencies to prevent and reduce street homelessness and increase sustainment of accommodation To work with individuals to build community ties through the engagement opportunities of work and volunteering with a view to moving them towards becoming economically active and empowered Manage allocation of personal budgets as appropriate To produce high quality casework management forms and maintain accurate records To assist in developing best practice in providing homelessness services and solutions to homelessness To participate in the delivery of local Rough Sleeper Counts including the annual rough sleeper count which takes place at night To ensure the appropriate management of client related risk, through the production of comprehensive and high-quality risk assessment and risk management plans and to take responsibility for the safe working of self and others and to ensure local procedures around Lone Working are followed To meet individual and team performance targets, including maintaining high standards of record keeping To foster and maintain relationships with internal and external partners and stakeholders to ensure the delivery of key outputs and performance indicators.
