The aim of this role is to reduce unnecessary social hospital admissions/ to support carer breakdown, and to promote independence, by assessing people in their own homes and assisting people to maximise independent living. Our focus includes community support before hospital transfer and assistance in Accident and Emergency departments. As a Level 3 Social Worker, you will assist the manager in delivering a holistic service, ensuring the safety of vulnerable adults, and facilitating prompt assistance for adults and their caregivers during crises such as falls or caregiver emergencies. Your responsibilities will involve assessing needs for eligibility, devising care plans, and achieving or maintaining well-being outcomes. We are seeking a motivated, organised Social Worker with energy and creativity. You will need to be registered with Social Work England, along with strong interpersonal and communication skills.
