Job Overview
We are seeking a highly motivated, innovative and experienced nurse to join our team. This role focuses on providing holistic health assessment and support to children and young people in care within Cheshire West and Chester.
Responsibilities
* Lone working visits to children and young people in their chosen settings (home, school, face-to-face or virtual).
* Conduct holistic health assessments and make referrals or recommendations suited to each young person’s needs.
* Work autonomously while receiving appropriate levels of supervision within the team.
* Liaise closely with Cheshire West and Chester’s Supporting Independence Team, dedicating 1 day per week to work alongside.
* Travel between service users and other services within the Cheshire West and Chester footprint; access to transport is essential.
Qualifications
* Degree in specialist community public health nursing or qualification recognised as equivalent by a professional body, with appropriate registrations.
* Experience working with children and young people in care, including children 16 years and over and unaccompanied asylum-seeking children.
* Interest in working with young people 16 year and over and ability to work flexibly between 8:30 am and 6 pm.
* Excellent communication skills and ability to work in multi‑disciplinary environments.
* Strong organisational and autonomous working style, supported by appropriate supervision.
