The role of the Rough Sleeping Services Pathway Manager is to ensure the efficient functioning of the vulnerable rough sleeping adult accommodation pathway by coordinating and facilitating all moves into and across services.
Oversee the triaging of referrals, assessing clients needs and referring clients to the most appropriate service to meet their needs whilst operating in and maintaining an open, transparent and fair process. Ensure that all aspects of the services are delivered to the highest standards and are fully compliant with both our organisational values and contractual requirements to ensure that client needs are met.
Maybe required to travel between services and other sites. The post holder maybe required to work evenings, weekends and occasional public holidays in order to meet the needs of the service users and stakeholders such as the Severe Weather Emergency Protocol (SWEP) activation from the GLA or other emergencies.
Ensure daily room checks are carried out and sign post clients for help and assistance for example hoarding, self neglect etc
