Job Summary
We are seeking a compassionate and dedicated Care Practitioner to join our team. The ideal candidate will play a vital role in delivering high-quality care to individuals in both home care and care home settings. As a Care Practitioner, you will be responsible for developing and implementing personalised care plans, ensuring that the needs of our clients are met with dignity and respect. Strong communication skills and the ability to drive are essential for this position.
Duties
* Develop and implement individualised care plans tailored to each client's needs.
* Provide personal care and support to clients in their homes or within a care home environment.
* Assist clients with daily living activities, including bathing, dressing, and meal preparation.
* Maintain accurate records of client progress and communicate effectively with healthcare professionals.
* Drive to various locations as required to deliver care services.
* Ensure a safe and clean environment for clients by adhering to health and safety regulations.
* Collaborate with multidisciplinary teams to enhance the quality of care provided.
Experience
* Previous experience in home care or within a care home setting is highly desirable.
* Familiarity with IT systems for record keeping and communication purposes.
* Proficient in English, both spoken and written, enabling effective communication with clients, families, and colleagues.
* A caring attitude with a commitment to providing high-quality support to individuals in need.
* A valid driving licence is preferred to facilitate travel between client locations.
