Job summary
Newhall Surgery is looking for a full-time / part-time (25 to hours) dynamic and motivated Practice Nurse to join our Nursing Team. The successful candidate will have experience in chronic disease management. A working knowledge and experience of LD reviews, LTC reviews, NHS Health Checks, ECGs, blood pressures, bloods, foot checks, urine analysis, spirometry, ear syringing cervical cytology, wound care and minor injuries management, general health and lifestyle advice, vaccinations, injections and immunisations would also be an advantage.

Main duties of the job
To manage treatment room consultations such asminor injuries, wounds, dressing and treatment of leg ulcers, suture removal,ear examination and syringing.
Administer routine injections in accordancewith the practice policy and appropriate protocols, PGDs and PSDs ie Childhoodand adult immunisations in accordance with national guidelines such as: Fluvaccinations, travel vaccines and completion of patient vaccination records.Anti-psychotic injections, Vitamin B12 injections and contraceptive injections.
Contribute to Chronic Disease Management inareas such as: Asthma, COPD, Diabetes, CVD Checks, Learning Disabilities, LongTerm Conditions and NHS Health Checks and to other Enhanced Servicespecifications as required.
Provide women's health services includingcervical cytology and advice on contraception and the menopause according topractice policy.
To undertake investigative proceduresincluding ECG, blood pressure, urine analysis, foot checks.
To prepare and assist with minor surgicalprocedures plus coil and implantprocedures.
Contribute to Nursing Team record keeping:clinical, administrative, H&S related records.
To assist in and perform routine tasks relatedto patient care as directed by GPs and the Management Team.
With support implement and evaluate individualtreatment plans for Chronic Disease patients that promote health and wellbeing.

Our clinical staff consists of 3 GP Partners, 4 GPs, 2 GP Registrars plus a dedicated Nursing Team, Pharmacy Team and a Health & Wellbeing Coach.

To be actively involved in health promotion to thePractice population in collaboration with the Primary Health Care Team, meetingthe needs of the patients and supporting the delivery of the Practice Policies& Procedures. The post holder willwork within their professional competence in accordance with the NMC Code ofProfessional Conduct and Scope of Professional Practice.
Towork under indirect supervision and perform tasks prescribed and delegated bythe GP Partners, Practice Manager and Nurses working to PSDs, PGDs, agreedprotocols, within an evidence base and within defined competencies to meet theneeds of patients.
To liaise with the Practice Pharmacy Team,Community Staff, Practice Nursing Team plus other healthcare professionals andto working closely with the Management, Administration and Care Navigationteams.
