Job summary
We are seekingan Advanced Clinical Practitioner, who has a passion for frailty, to join our CareHome team based in Care Home Team undertakes the reactive andproactive care for patients in Residential Care and Nursing Homes within thelocality working in partnership with the PCN colleagues and alongside dedicatedGP support. The successful candidate will join the established team in workingtogether to ensure the delivery of high quality care for people within localCare homes.
As an experienced practitioner you will havecompleted the Advanced Assessment Skills and Non-Medical Prescribing (V300)modules and will hold the full Masters qualification in advanced practice asreflected in the attached JD
Main duties of the job
Theability to work independently is essential as the post holders will need tooffer urgent care, treat minor illness, give patient advice and offer chronicdisease management. You will have the ability to prioritise and triage theneeds of patients, making any referrals for further investigations whereappropriate in line with local and practice guidelines, whilst ensuring yourclinical practice is safe and effective; you will be fully supported within acollaborative, multidisciplinary team.

Job responsibilities
As an ACP,you will work with a high degree of autonomy, demonstrating core capabilitiesin clinical competency, encompassing the four pillars of clinical practice. Youwill treat patients with acute exacerbations of chronic diseases, manage longterm conditions and assess, plan, and treat patients to prevent them beingadmitted to hospital. You should have excellent clinical, written,and verbal communication skills, be able to work independently and manage yourown workload. For purposes of the role, you must be NMC registered, hold a fullUK driving licence, have good IT skills and have excellent communicationskills.
