An opportunity has arisen for an enthusiastic and highly motivated social worker to join the innovative Barts Health NHS Trust Young Adults Diabetes Service. This mobile service will comprise a Clinical psychologist, Dietician, Diabetes Specialist Nurse, and Youth Worker.

This exciting new post has been developed to improve diabetes care and outcomes for young people (age 16-25) with diabetes across North East London. The post holder will support young adults and their families who may be struggling with a range of difficulties related to living with diabetes, as they make the transition from paediatric care to adult services. The work will be young adults living with both Type 1 and Type 2 diabetes.

As this is a new post, the post holder will have scope to develop the service, with the support of more senior colleagues and in collaboration with service users, and to take a leading role in the future of diabetes services for young adults across North East London.
The post will involve cross site working across Barts health and North East London communities: Mile End Hospital, Whipps Cross Hospital, the Royal London Hospital, and St Barts Hospital.

Main duties will include

· Build strong working relationships with all members of the multidisciplinary healthcare team, both verbally and in writing.
· Be the primary point of contact for Children and Young People and their parent/carers accessing social care services, arranging meetings by phone or in person as required.
· Develop and sustain good working relations with stakeholders, other agencies working with children and young people including education and the voluntary sector to identify relevant non-clinical interventions.
· Attend staff meetings, supervision and training as required, internal to Barts Health and external as required.

In addition to being a member of the core young adult multidisciplinary team (MDT) the successful candidate will join the wider Barts Health diabetes service, and will be well supported in this highly-regarded service, by a more senior clinicians.

Main duties of the job

The postholder will be establishing and testing a model of social work based within the hospital setting, linking to voluntary and local authority organisations.

The post holder will work with the the health care team to co-ordinate and deliver a programme of support for young people (16-25 year olds) with diabetes.

This is an exciting opportunity to get involved in a project in its early planning stages, and to have a real impact in bringing to life the first SocialWorker service for Young People with diabetes using Barts Health services.

Essential criteria

1. Diploma level qualification in child development/youth work/education
2. Postgraduate qualification in social work

Experience

3. Knowledge of health and mental health
4. Experience of working with children and young people in a person-centred way (for example, in health, social or community capacity)
5. Committed to improving lives and communities
6. Excellent interpersonal skills with experience of working with people from diverse backgrounds. Ability to build rapport with a wide range of young peopledemonstrating empathy and understanding
7. Experience of working within a framework of confidentiality and with access to sensitive personal data
8. Excellent IT skills including previous use of Microsoft Office and CRM database systems
9. Experience of demonstrating impact and user outcomes
10. Ability to develop and maintain relationships with professionals and voluntary sector providers across the borough
11. Enthusiasm, flexibility and a positive ‘can do’ attitude

Desirable criteria

12. Experience of working with YP with physical/mental health conditions
13. Knowledge of how chronic health and mental health can impact YP functioning
14. Existing knowledge of local services and resources
15. Understanding of issues affecting children and young people and understanding of self-care principles
16. An understanding of Health and Social Care Services
17. Must have ability to undertake own transport e.g.bicycle, use public transport and able to travel efficiently across the borough
