Overview
We are looking to recruit an SCPHN qualified School Nurse to join our enthusiastic and innovative School Nursing team. This post will be based at Hobmoor Road Health Centre, 192 Hobmoor Road, B10 9JH.
The post holder will be responsible for leading a multidisciplinary team of school health staff who will work with children, young people and families to identify relevant health needs and ensure these are addressed in line with the agreed service pathways. You should have strong clinical leadership, public health knowledge, and excellent interpersonal skills to work with teams and communities.
Main duties of the job
* Assess the health needs of individual children and the school population within your allocated district and support other districts as needed.
* Proactively address factors affecting health to improve outcomes for children and young people and reduce health inequalities and social exclusion.
* Lead a multidisciplinary team of school health staff and develop multi-agency partnerships at school and district level.
* Manage and mentor nursing colleagues in the delivery of health training programmes for schools within the caseload.
* Increase the visibility and accessibility of school health services through engagement with schools, pupils, partner agencies and the voluntary sector.
* Mentor pre-registration students and provide long-arm mentoring for post-registration students in line with BCHC guidelines.
* Undertake holistic assessments of children and young people, write and deliver care plans, and delegate work appropriately.
* Contribute to multi-agency interventions for children subject to safeguarding, Early Help or SEND.
* Develop school partnership agreements and district profiles, using demographic data to plan services that meet needs at the individual, school and community level.
* Act as a liaison between children, teachers, parents, medical officers, primary care teams and others; carry out home visits as required.
* Deliver services in settings beyond schools as necessary and lead allocation meetings to distribute work equitably.
* Maintain accurate records and prepare reports, ensuring confidentiality, and support other team members in record-keeping.
* Participate in case conferences, case reviews and core groups; write court reports in conjunction with legal services.
* Provide cover for colleagues as designated by the Team Leader; support medicines in schools policy; participate in clinical governance and audits.
* Be proficient with software used in clinical care and ensure data is entered timely and accurately, including Rio electronic patient records or other systems as required.
* Engage in the ChatHealth text messaging service and work flexibly up to 6pm on weekdays with occasional weekend work as needed.
* Take on the role of duty nurse and lead at allocation meetings.
Essential qualifications and experience
* Specialist Community Public Health Nurse – School Nursing qualification or currently working towards completion (offer conditional on successful qualification).
* Evidence of post-registration development and training.
* Registered nurse with Specialist practitioner degree in school nursing or equivalent
