The Role:
As a Relief Support Practitioner, your responsibilities include:
* Supporting independent living at home
* Providing personal care and daily activity support
* Meal preparation and nutritional support
* Offering emotional support and companionship
* Preventing hospital admissions proactively
You will work across various services, with shifts including evenings, weekends, and nights, offering flexibility.
