JOB DESCRIPTION : Lifestyle Co-Ordinator (Community) ACCOUNTABLE TO: Service Manager (Community) HOURS: 37hrs per week Plus including out-of-hours On Call (additional payment) SALARY: Depending on experience and qualifications. Values Diversity, Aspiration, Choice and Togetherness Purpose of the Role To lead and manage a community-based support service for disabled adults, delivering high-quality, person-centred care that promotes independence, wellbeing, and choice. The post holder will ensure the service is safe, effective, caring, responsive, and well-led, in line with CQC standards and the Care Act 2014, while embedding the values of Thurrock Lifestyle Solutions CIC: Diversity, Aspiration, Choice, and Togetherness. Key Responsibilities 1. Leadership and Team Management Manage a team of Personal Assistants, ensuring effective delegation, supervision, and performance through regular appraisals. Organise and maintain staff rotas that meet the needs of Thurrock Lifestyle Solutions CIC customers. Participate in the Senior Management Team (SMT), attending monthly meetings and contributing to strategic planning and service development. Promote a culture of continuous learning and professional development across the team. 2. Person-Centred Support Support customers to co-design, implement, and evaluate their Individual Lifestyle Plans with measurable outcomes based on their goals. Empower customers to access community resources and opportunities that promote independence, dignity, and social inclusion. Ensure all support is delivered in line with the Mental Capacity Act, safeguarding protocols, and CQC’s “Right Support, Right Care, Right Culture” framework. 3. Communication and Reporting Maintain a high standard of written and spoken English. Write professional reports, attend planning sessions and reviews, and contribute to multi-agency meetings. Use IT systems effectively for care planning, documentation, and communication. 4. Compliance and Quality Assurance Ensure full compliance with relevant legislation including the Health and Safety at Work Act, Equality Act, and CQC regulations. Participate in audits, inspections, and service evaluations to maintain and improve quality. Be part of the On-Call rota system to provide emergency out-of-hours support and guidance. 5. Collaborative Working Work in partnership with health and social care professionals, families, and unpaid carers to ensure holistic and coordinated support. Promote positive relationships and effective communication across all stakeholders. 6.
