Location: Hybrid – Home and field based across Mid and South Nottinghamshire
Hours: 37 hours per week
Salary: £24,915.80 per annum (AUNN Band E)
Tenure: Full Time,
Contract: Fixed term until 30th September 2026

Senior Community Support Worker – The Role:

We’re looking for a Senior Community Support Worker to join our Connect Service – a short-term support service designed to help adults maintain independence and improve health and wellbeing. The service prioritises older adults and those with long-term conditions, learning disabilities, autism, neurodiverse conditions, and acquired brain injuries.

Senior Community Support Worker – Key Responsibilities:
- Act as a key point of support for our Support Workers, offering guidance, mentoring, and leading reflective practice sessions
- Lead on triaging new referrals, ensuring timely and appropriate responses to individuals’ needs
-Provide direct support to individuals via telephone and face-to-face contact, helping them to identify solutions and build confidence in managing their own wellbeing
Senior Community Support Worker – You:
- Empathetic, sensitive, and confident in working with vulnerable adults
- Self-motivated and disciplined, with the ability to manage a busy caseload while working from home
- A strong communicator and team player, with leadership potential or experience
- Is able to work in a hybrid way, travelling across Mid and South Nottinghamshire as required
