Overview
The applicant will be applying for the position of support worker within the Memory Assessment Service in the Rhondda Valley.
The Support Worker will act as an expert resource providing specialist advice and support to clients and carers after a diagnosis of Dementia.
Working collaboratively across a range of multi-disciplinary services and agencies.
To become a named contact for clients and carers following assessment and diagnosis at the memory clinic.
They will also complete home visit to clients that are needing additional support and signposting to other services.
Build the clients confidence to attend social groups.
They will attend community groups to network and by a contact when required.
Responsibilities
* Support the provision of Health and Social Care to people living with dementia in the community within the catchment area of Cwm Taf Morgannwg University Health Board.
* Work alongside a MDT and multi-agency team.
* Work without direct supervision and maintain own workload and safety.
* Assist in organization, facilitation and running of groups within community and hospital settings.
* Participate in caseload management supervision and continuous professional development.
Qualifications
Welsh Skills Desirable: Level 3 (equivalent to CEFR B2) or above speaking & listening.
