Social Prescribing Link Worker for the East Herefordshire Primary Care Network (PCN).
Primary duties: personalised support to patients, connecting them with services that meet their non‑clinical needs.
Key Duties
* Undertake holistic assessments of patients’ circumstances to determine the nature and extent of their non‑clinical needs.
* Co‑design relevant actions with patients, ensuring maximum engagement in improving health and wellbeing.
* Introduce or reconnect people to community groups and statutory services.
* Develop and maintain partnership relationships with statutory and voluntary sector organisations to deliver joined‑up service provision.
* Work with practices and the PCN team to proactively target local health inequalities and poorer health outcomes.
* Provide patients with a coordinated experience of care, promoting choice, control, empowerment, independence and resilience.
* Co‑produce simple, personalised care and support plans for individuals, families and carers.
* Deliver group sessions targeting patients with particular health needs to reduce inequalities and improve outcomes.
* Maintain an up‑to‑date database of community resources to support the social prescription menu.
* Act as an advocate for the social prescribing service across the PCN, public and professionals.
* Gather and share good practice on social prescribing services.
* Participate in programme development, monitoring and evaluation, providing reports to the PCN manager and team.
* Provide accurate, professional feedback to referrers upon request.
* Maintain manual, records, calendars and diaries for patient appointments and related administrative tasks.
* Work within safeguarding, equal opportunity and confidentiality frameworks.
* Comply with organisational policies, quality assurance and training requirements.

* Salary: £29,541.82 to £34,086.72 per year

* Job locations: Ledbury Health Partnership, Colwall Surgery (and other GP practices in the East Herefordshire PCN)
