Social Worker - Adults Multi-Disciplinary Team
Social Worker – Mental Health Team within Newham: earn £32.01 hourly in this rewarding and ongoing full-time role. What you will do
* Conduct comprehensive community care assessments including Care Act assessments
* Undertake capacity assessments and contribute to tribunal processes
* Engage in safeguarding assessments to ensure the protection of vulnerable individuals
* Facilitate hospital discharges to help clients transition smoothly back into their communities
* Collaborate closely with a team of mental health professionals to provide quality care and support
