Job Title: Social Worker - Older Persons and Physical Disability\nLocation: Southwark (Hybrid)\nPay Rate: £33.76 per hour\n\nLiquid Personnel is seeking a compassionate and dedicated Older Persons and Physical Disability Social Worker to join the team. The successful candidate will provide support, advocacy and care coordination for older adults and individuals with physical disabilities, helping them to achieve their full potential and improve their quality of life.\n\nResponsibilities :\n\nConduct comprehensive assessments of older adults and individuals with physical disabilities to identify their needs and develop personalized care plans.\nWork closely with healthcare professionals, caregivers, and other social services to coordinate care and support for older adults and individuals with physical disabilities.\nRespond to crises and provide immediate support and intervention to individuals and families in need.Benefits : \n\nSupportive manager and team\nHybrid workingQualifications and Experience : \n\nTo be successful in this role, you must be registered with Social Work England, be eligible to work in the UK, and hold a full UK driving licence.
