Summary:
The Care Navigator is a non-qualified role pivotal to the delivery of the Council's Adult Care and Health strategy, with a core focus on prevention, independence, and community-led support.Operating at the front door of social care, the role is responsible for being the first point of contact for vulnerable adults, older people, and carers. The primary purpose is to provide high-quality information, advice, and guidance, and to effectively 'navigate' individuals to the most appropriate services within the local community, voluntary sector (VCSE), and statutory services, in line with a person-centred and strengths-based approach.The Care Navigator ensures the legal framework of the Care Act 2014 and the principles of the Mental Capacity Act 2005 are upheld by promoting choice, control, and maximising the independence of Wirral residents.
Responsibilities:
 * Serve as the initial point of contact for enquiries, conducting effective triage to understand the individual's needs and goals. Signpost and refer individuals to appropriate community assets, voluntary organisations, and preventative services (often referred to as 'Social Prescribing').
 * Maintain accurate, timely, and up-to-date records of all client interactions, referrals, and outcomes using relevant social care and health database systems (e.g., Liquid Logic, EMIS).
 * Assist people in accessing formal Adult Social Care assessments (e.g., for financial assessment or statutory support) and carers' assessments, liaising with qualified Social Workers and other multi-disciplinary team members to ensure a smooth pathway of care.
 * Proactively develop and maintain a strong working knowledge of the local community offer, building effective relationships with the Voluntary, Community, and Faith Sector (VCFS), as well as internal Council and NHS services.
Essentials:
 * GCSEs (or equivalent) including English and Mathematics.
 * Adult Safeguarding training (or willingness to complete immediately).
 * Proven experience (e.g., 6+ months) of working within a Health, Social Care, or voluntary sector setting, in direct contact with the public.
 * NVQ Level 2 in Health and Social Care, or a relevant Customer Service Qualification.
 * Completion of a Care Certificate or Active Signposting/Care Navigator training.
