About The Role

Role Overview
What would you do if you had no family to support you when you were in need and in hospital? Or if you were in hospital and could go home but there was no one to take you?
Some people are lucky to have family and/or friends to help, or good neighbours who will pop in to see them. These are the people we support, the ones who just want to go home. This is where we can help. We take people home from hospital, assess their needs, and offer support and guidance between & hrs.
The hospital wards will refer a patient to us, possibly asking us to take the patient home, ensure the heating is on, and that there is food in the fridge prior to discharge. You might be asked to collect prescriptions, make sure they have food at home, perform light household tasks, and ensure they know who to contact if they need more help. The support you provide should be what you would want your own friends and family to experience.
This job is about helping patients discharged from hospital have a safe journey home, ensuring their home is livable, and they have everything they need, reducing the risk of readmission.
Your days will involve speaking to and supporting patients and medical staff to help the hospital run smoothly and assess patients efficiently.
Your experience will help people regain independence after hospital stays. The individuals you support come from diverse backgrounds. This role will have a significant impact on our service users’ lives.
Note: This role does not involve personal care such as bathing, cleaning, or administering medications.
A Day in the Life of a Support Worker
* Communicating with healthcare professionals about patient support needs
* Driving to various locations including hospitals, GP surgeries, pharmacies, shops, and patients’ homes using BRC vehicles
* Providing emotional and practical support to service users
* Assessing future needs and developing support plans, signposting to relevant agencies and community groups
* Updating systems to track support provided
Requirements to be Successful
* A manual driving licence
* A caring, empathetic nature
* Basic IT knowledge, with training on British Red Cross systems provided
* An understanding of NHS services to offer appropriate advice and guidance
