The post holder will provide primary support to general practice staffwith regards to prescription and medicines management. They will help supportthe repeat prescription system, deal with acute prescription requests, andmedicines reconciliation on transfer of care and systems for safer prescribing,providing expertise in clinical medicines advice while addressing both publicand social care needs of patient in the GP practice.
They will deliver an efficientand high quality clinical service by providing clinical and professionaldevelopment to the clinical workforce within Westway Medical Centre. The post holder will ensure that the practice improves patient outcomes,ensure better access to healthcare and help manage workload. The role ispivotal to improving the quality of care and operational efficiencies sorequires motivation and passion to deliver excellent service within generalpractice
The post-holder will deliver a comprehensiverange of high quality pharmaceutical services, to include the assessment of patientsin practice and remotely. They will be able to competently consult on thetelephone, via the internet and face to face. They may occasionally be requiredto complete home visits.
