We’re now recruiting an Outreach Team Leader to provide strong, hands-on leadership across community-based and home support services.
About the Role
As an Outreach Team Leader, you’ll lead from the front, delivering direct support while overseeing the day-to-day care an support provided in people’s homes and within the community. You’ll support and guide a team of support workers to deliver safe, consistent, and person-centred care tailored to each individual.
Working closely with the Registered Manager and other senior colleagues, you’ll ensure care plans are up to date, risks are well managed, and staff are confident in supporting people respectfully and professionally within their own homes.
Key Responsibilities
* Lead, coach, and motivate support workers delivering care in people’s homes
* Ensure the people we support are at the heart of everything we do, promoting independence, choice, and inclusion
* Support individuals to live safely and confidently within their own homes and communities
* Coordinate staff rotas to ensure continuity and appropriate coverage
* Carry out staff supervisions, appraisals, and team meetings
* Support recruitment, inductions, and on-the-job mentoring
* Maintain accurate care plans, risk assessments, and daily records in line with CQC standards
* Oversee medication management, finance auditing, and health & safety checks
* Contribute to service development and continuous quality improvement
* Build positive, professional relationships with families, social workers, and other professionals
* Participate in on-call duties, and weekend working
About You
You’ll be a calm, confident, and compassionate leader with a passion for supporting people to live independently in their own homes. You’ll bring strong organisational skills, emotional intelligence, and a supportive leadership style.
Essential skills & experience:
* Experience as a Senior Support Worker or Team Leader within social care
* Strong understanding of learning disabilities, autism, and mental health needs
* Clinical experience with tracheostomy care, nebulisers, cough assist, epilepsy management, PEG feeding is highly desirable
* Experience supporting people in their own homes or community-based settings
* Experience coordinating rotas and supporting staff teams
* Excellent communication, organisation, and problem-solving skills
* Flexible to work occasional evenings and alternate weekends
* Full UK driving licence and access to your own vehicle (essential)
