Job Summary
Part‑time Senior Community Nurse (band 6) – 18 hours per week, working evenings and nights (17:30‑23:30) over a 7‑day rota, based at Droitwich Medical Centre.
Responsibilities
* Deliver independent nursing care in patients’ homes, preventing hospital admission and supporting early discharge.
* Take direct referrals from 111, ambulance services and discharging hospitals; triage and allocate work via handheld tablet.
* Manage a diverse caseload – catheters, palliative care, wound care, medication administration and rehabilitation from hospital discharge.
* Collaborate with the Evening & Night team, rotating into the triage role at Droitwich at least once a week.
Qualifications
* Registered Nurse (RN1) – graduate or working toward a Level 6 qualification.
* Preceptorship training and evidence of ongoing professional development.
* Experience in a community setting with transferable clinical experience.
