Job Title:Adult Social Worker – Community Team

Salary: £41,511 – £44,711 per annum

About the Role:
We are looking for an enthusiastic and dedicated Adult Social Worker to join the Community Team, providing support to adults with care and support needs. This role involves working directly with individuals, their families, and multi-agency professionals to assess needs, develop care plans, and promote independence in the community.
The successful candidate will be responsible for completing statutory assessments, risk management, and safeguarding interventions under the Care Act 2014 while working collaboratively with health and voluntary sector partners to ensure holistic support for service users.
Key Responsibilities:
* Undertake Care Act 2014 assessments to identify individual needs and determine eligibility for support.
* Develop personalised, strengths-based care plans, supporting individuals to maximise independence.
* Ensure people have access to direct payments, personal budgets, and community-based services.
* Conduct Mental Capacity Act assessments, ensuring best interests decisions are made when necessary.
* Identify safeguarding concerns and take appropriate action in line with Section 42 of the Care Act 2014.
* Work with adults at risk of harm, ensuring they are protected and supported effectively.
* Undertake risk assessments, ensuring care plans are responsive to changing needs.
* Collaborate with health professionals, housing teams, voluntary organisations, and community services to deliver coordinated care.
* Attend multi-disciplinary team (MDT) meetings, case reviews, and safeguarding meetings.
* Advocate for service users, ensuring their voices and choices are heard in decision-making.
* Maintain accurate case records, ensuring compliance with Care Act 2014, Mental Capacity Act 2005, and Human Rights Act 1998.
* Ensure all work aligns with local authority policies and best practice frameworks.
* Provide reports for reviews, court proceedings, and funding panels, where required.
Qualifications and Experience Required:
* Degree in Social Work (or equivalent) from a UK university
* Registration with Social Work England
* Post-qualifying experience in adult social care, hospital discharge, or community-based care planning.
* Strong knowledge of Care Act 2014, Mental Capacity Act 2005, and NHS Continuing Healthcare (CHC) frameworks.
* Ability to manage complex casework and risk assessments.
* Strong problem-solving and decision-making skills.
* Experience working in a multi-agency environment, ensuring a coordinated approach to care.
