Reablement and Falls Specialist Physiotherapist
Job summary: The post holder will be based at Monnow Vale Health & Social Care Facility, Integrated Service Hub for Central Monmouthshire, working alongside health & social care colleagues in a well-established community-based Integrated Team. The post holder will lead on the development & delivery of a specialist falls service for Tier 3 Complex Fallers, engaging the wider MDT, and working alongside existing qualified and support staff, and support delivery of a physiotherapy service to Tier 2 Fallers requiring Reablement input. The post holder will additionally manage a general community-based caseload as required. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
* Provide advanced clinical falls risk assessment, clinical reasoning and diagnostic skills for older people with a wide variety of complex falls presentations in community settings
* Devise bespoke intervention management and rehabilitation programmes for specialist caseload based on assessment findings (may include vestibular assessment and treatment and CBT for service users fearful of falling)
* Independently manage a highly specialist complex caseload, making differential diagnoses and advising on further medical intervention/investigation as required and in a timely manner
* Provide a wide range of clinical and leadership skills to support management of the Reablement Physiotherapy caseload and direct management of Tier 2 fallers

Job Description and Qualifications
* Essential: Diploma/degree in Physiotherapy; Registered with Health Professions Council (HCPC)
* Desirable: Postural Stability Instructor qualification (PSI)
* Registration: Must be registered with HCPC; current UK professional registration
* Desirable: Experience of working alongside GPs/consultants and MDT/IST members with this specialised caseload
