Adult Social Care and Prevention Services
1 x 37 hours per week, permanent post based at the RVI Hospital
We have a social work opportunity available. Our job is to help people get home or where they need to be as quickly and safely as possible and then work with them after discharge to maximise their independence. The work is short-term in nature, intensive and very rewarding.
We could list what we want from you, but you can find that out from the person specification. Instead, we asked one of the current post‑holders to tell you why you should apply:
"This is a dynamic role where every day brings new opportunities to learn and develop. You feel part of a good team which helps to make everyday challenges more sustainable. The team are a good supportive team, offering encouragement and reassurance to each other, giving a happy, positive atmosphere within the office. The team members build upon each other’s strengths and challenges by sharing skills and knowledge they have built from other networks and positions they have come from. You get the chance to build knowledge across NHS Continuing Healthcare, working alongside other professionals, mental health, housing, dementia, therapy teams and more, helping you become a well‑rounded practitioner whilst supporting people through tough times. We are often involved with people who find themselves in a crisis, and it makes you proud to be able to assist them through their journey while in hospital. The job is made easier thanks to excellent managers who are always there to guide you and encourage you in everything you do. They have an open‑door policy which makes it easier for discussions around cases. With every hospital admission, lives are reshaped, and you have the privilege of being a steady source of support during those challenging moments. We manage the crisis which has brought people into hospital, reassuring people of the uncertainties of what comes next, guiding them and supporting them when discharged to enable them to maintain their independence."
