Overview
We are expanding our Rapid Response Community Nursing Team in Luton to support the delivery of community urgent crisis response requirements and the development of Virtual Wards in the community. This presents an opportunity for experienced and skilled Registered Nurses to join the team. The Rapid Response Team in Luton is an established and successful team providing high quality nursing care to maximize health status, minimize the effects of ill health and disability, and enable independent living. The team collaborates with the acute hospital, ambulance service, GP practices, and other local providers to deliver care at home that avoids unnecessary hospital admission; it also supports patients receiving District Nursing by providing unscheduled/urgent care in and out of hours across the Luton area. Shifts cover 08:00 to 21:00.
As a senior member of the team you will lead by undertaking clinical assessments, planning and delivering care, demonstrating effective leadership, and contributing to continuing service development to meet business and operational needs. The post may close early if a high number of applications are received after 18 September.
Responsibilities
* Provide clinical support and leadership to Community Rapid Response services; contribute to the ongoing development of the service in line with local priorities and national strategies.
* Provide acute clinical assessment and treatment to patients in their own homes, including residential homes, planning, assessing and evaluating patient care packages and arranging onward referrals as required.
* Liaise with community, primary and secondary healthcare professionals to ensure a holistic care package designed to meet individual needs, prevent unnecessary hospital admissions, and facilitate timely discharge.
* Work with the Team Lead to provide leadership and line management support to team members to deliver safe, effective care; ensure pathways and processes support safe care.
* Contribute to monitoring and development of local policies, protocols and procedures to provide safe, effective telephone assessment and prioritisation of referrals.
* Develop specialised care packages for patients; plan and deliver prescribed therapy in patients’ homes, including intravenous therapy as prescribed.
* Coordinate discharge planning with Discharge Planning and medical staff; respond to and plan packages of care with GPs and the multidisciplinary/community nursing team to avoid inappropriate admissions.
* Ensure appropriate staff cover for caseload management and service delivery; provide Clinical Nurse Triage.
* Recognise and manage complications in patient care and refer to physicians/GPs/Specialists as required; prioritise workload and allocate visits accordingly.
* Educate patients on treatment nature, side effects and expected outcomes to empower self-management; where applicable, prescribe medications in line with independent nurse prescribing guidelines.
* Identify and manage crisis situations; escalate risks and actions to senior managers as required.
Person Specification
Qualifications and Training – Essential
* RGN / ECP / Paramedic (HCPC) with current registration
* Educated to degree level or equivalent
* MSPP or equivalent
* Evidence of recent continuing clinical and professional development
* Independent Nurse prescribing course
* Commitment to undertake appropriate education at Masters level
* History taking or equivalent
Qualifications and Training – Desirable
* Management qualification
* Specialist postgraduate course in relevant subject
Experience – Essential
* Experience in team management, resource management and workload coordination
* Performance management
* Significant post-registration experience including community experience
* Work across organisational boundaries and multidisciplinary care coordination
