Community Engagement Worker – Annual Health Checks
Location: West Midlands Region (Community Based)
Hours & Salary: 30 Hours Per Week – £29,400 (Pro-rata)

About
We’re seeking a compassionate and proactive Community Engagement Worker who is passionate about improving healthcare access for people with learning disabilities.
Responsibilities
* Community Engagement & Outreach: Work directly with individuals with a learning disability, their families, and carers to promote awareness and understanding of Annual Health Checks; deliver inclusive sessions in community venues, day services, support hubs, and public spaces; provide one-to-one support and peer-informed outreach to build trust and increase confidence in using health services.
* Promotion, Accessibility & Awareness Campaigns: Co-design accessible promotional materials (including Easy Read, visuals, and digital content); promote Annual Health Checks through outreach events, local media, social media, and community champions; ensure messaging is tailored to meet the cultural and communication needs of diverse communities.
* Partnership Development: Build and maintain relationships with Primary Care stakeholders (GPs, practice staff, PCNs, Learning Disability Health Facilitators), statutory agencies (ICBs, NHS community teams, local authorities), and voluntary, community and faith-based organisations; facilitate aligned working and shared campaigns for inclusive messaging.
* Steering Group Collaboration: Work with the project Steering Group, participate in regular meetings, and use insights to shape delivery; ensure people with lived experience have genuine input into planning, materials, and evaluation; act as a conduit for feedback between the Steering Group and wider community voices.
* Monitoring, Learning & Impact: Record engagement activity, participant data, and feedback using agreed tools; collect real-life stories and case studies to demonstrate impact; contribute to written reports, dashboards, and presentations for funders and partners; support evaluation and reflective learning to inform future delivery.
Qualifications
* Essential: Experience of working with people with a learning disability or additional needs; strong communication and interpersonal skills to engage across diverse communities; experience of partnership working or collaborative project delivery; knowledge of health inequalities and barriers to healthcare access; ability to deliver accessible outreach using a range of tools; commitment to inclusive practice, co-production, and empowerment.
* Desirable: Experience in Primary Care, social prescribing, or community health; knowledge of Annual Health Checks and the UK primary care system; experience of working with or supporting a project steering group or advisory panel; lived experience of disability or caring for someone with a learning disability or autism; full driving license and access to a car (desirable) and willingness to commute within the area and obtain business insurance.
