Job summary
West Kent Primary Care are seeking to employ a Frailty and Care Homes Clinical Pharmacist on behalf of Tunbridge Wells Primary Care Network (PCN). The post holder will work in the care homes team to provide improved qualityand safety of care offered to patients in care homes. They willhave regular contact with care home residents, aim to reduce GP workload incare homes and will support the management of patients with long-termconditions.
Tunbridge Wells Primary Care Network(PCN) is made up of 8 member GP practices servicing over 85,000 patients. We are a forward thinking, dynamic andcohesive PCN offering services including First Contact MSK Assessment, SocialPrescribing, Paramedics and Clinical Pharmacy.
Main duties of the job
The Care Home Clinical Pharmacistwill improve the quality and safety of care offered topatients of Tunbridge Wells who are living locally in care homes or arehousebound. Theywill conduct medication reviews at care homes or in patients homes, buildingeffective relationships with all care home staff and carers. They will have regular contact with patients and will reduce theworkload of GP prescribers in the practices, supporting a multidisciplinary team with structuredmedication reviews, treatment escalation plans and support the management of patients with long-term conditions.
As part of this role the PCN willfund for you to be enrolled into the NHS England CPPE Education Programme whichlasts for 18 months. Study leave and time off to attend courses will be builtinto your work plan.

9. Provide support on medicines related issues to the practice andcare home teams
10. Develop Care Home services indiscussion with the Care Home GP Lead and Care Home Pharmacy Team
11. Managing workflow of residentsreviews
12. Lead in the ongoing care ofcomplex residents
13. Performface-to-face medication reviews of polypharmacy patients
14. Provideleadership on quality improvement and clinical audit, as well as managingaspects of medicines safety and antibiotic stewardship.

Essential
15. Previous experience as a community / acute / primary care based pharmacist
16. Experience in providing clinical advice and expertise on medicines to colleagues and patients

17. Knowledge of structured medication reviews and producing recommendations for prescribing and monitoring
18. Knowledge of medicines management at point of hospital discharge including patients living in care homes
19. Experience of managing long-term conditions in a clinical setting
