Overview

An exciting and challenging opportunity has arisen for an enthusiastic and driven Band 5 Mental Health Practitioner to work within Early Intervention Team for Psychosis Liverpool. The service works with people between the ages of 14 - 65 who are experiencing a first episode of psychosis.


What we are looking for

We are looking for qualified Registered Nurse (MH), Occupational Therapist or Social Worker to join the team as a Key Worker. You will be involved in the patient journey from triage, assessment through to offering interventions supporting service users to recover from psychosis over a 3 year period. The role will include providing high quality evidenced based care, formulating risk management plans, supervising risk assessments, providing expertise and advice, care co-ordination and directing unqualified staff.


Responsibilities

* Coordinate the care of service users carrying out assessment planning, implementation and evaluation of their care under the guidelines of the care programme approach and under the supervision of the line manager.
* Ensure the service users care plan is developed to meet the assessed need of the individual and that unqualified staff are delivering care as laid down in the care plan.
* Liaise with family members, carers and significant others when completing the assessment and developing a care plan.
* Develop a risk management plan for service users, taking note of the contributions from other members of the team; ensure all significant people are aware of the plan and act accordingly.
* Direct unqualified staff to ensure delivery of a high standard of care to service users.
* Participate in setting standards for care based on sound evidence and research findings.
* Participate in systematic monitoring and evaluation of service users under guidance from a supervisor.
* Communicate with other members of the multi-disciplinary team to ensure information relevant to the service user’s care is shared.
* Use verbal and non-verbal communication skills to impart sensitive information; check understanding where barriers exist due to distress, psychiatric symptoms or language.
* Use training and experience to give and receive information about a service user’s care in reviews and multi-disciplinary meetings; these individuals may be external to the Trust.
* Have knowledge of the Mental Health Act, Mental Capacity Act and other relevant legislation; support service users and their carers when the Mental Health Act is used to admit the service user to hospital; ensure rights are communicated and outcomes documented.
* Provide and co-ordinate a range of therapeutic activities for service users and monitor their effectiveness.
* Take responsibility for the safe administration of medication in line with Trust policies and Nursing & Midwifery Council regulations.
* Ensure accurate documentation of information regarding service user care and treatment in the health record; supervise entries made by unqualified staff.
* Under supervision, provide written and verbal reports as requested (e.g., for Mental Health Act Tribunal or Managers Hearing).
* Contribute to teaching sessions to pass on knowledge and skills.
* Use skills to de-escalate situations where service users become physically or verbally aggressive; ensure unqualified staff receive appropriate support following incidents.
* Document all incidents or near misses as per Trust policy and follow up actions as required.
* Participate in service user reviews to ensure needs are met and the team has relevant information for clinical decisions.
* Develop care plans to help service users reach and maintain optimum health and independence, enabling them to remain in the community.
* React proactively in cases of service user crisis and implement immediate plans with supervision and guidance as appropriate.
* Assist in the assessment and supervision of nursing and other students.
* Assess and monitor the physical and mental health of service users, seeking medical advice as appropriate.
* Participate in team meetings to influence team working and the development of evidence-based practice.
